Transition Planning
What is transition planning?
Transition planning is to help you get ready for adult life.
You may decide to:
- Stay on at sixth form.
- Go to college.
- Go in to training.
- Get a job.
- Do voluntary work.
In Year 9:
- This is an important year for you, you will start looking at your options post 16.
- You will have a transition review with the people who are helping you to plan your future.
- You will start your transition plan.
- This will be updated every year.
In Year 10:
- You will work with your Connexions Personal Adviser looking at the world of work.
- Start thinking about your skills and what you can do.
- You may go on work experience either in Year 10 or Year 11 to give you some job ideas.
- You will have an annual review to look at what you want to do when you leave school.
In Year 11:
- This is your last year of compulsory education.
- You will work with your Connexions Personal Adviser to decide what you plan to do and what other support you may need to make a successful transition.
In Year 12/13
- You will look at independently living and look at what other support you may need to help you in the future.
- Your parents/carers will make links with the Adult Learning and Disability Team.
- You will get the opportunity to gain more information about the world or work.
- You will continue to have reviews.
- You will work with Connexions Personal Adviser.
In Year 14
- You will have your last review.
- A section 139a is completed, this tells us what support is needed to help you make the transition into adult life.
Make sure you plan for review meeting
Think about:
- What subjects you wish to study.
- What activities you would like to do while at school
- What information you need to make the transition.
- What support you may need to help you become independent.
